The undersigned parent/legal guardian responsible for payment, hereby;

1.         Give our consent that my child may attend ballet lessons.

2.         Agree to pay the applicable fee during the first week of the term.

3.         Understand that there will be no reduction in fees if my child fails to attend classes due to illness or for any other reason.

4.         Agree to inform Anma School of Ballet, in writing if my child terminates her ballet lessons.  A month’s written notice must be given, otherwise I will be held liable for a full term fees.

5.         In the event of Anma School of Ballet having to take legal action in terms of

Payment, the parents/guardians undertake to pay all legal costs incurred by the School on an attorney and client scale.

6.         Understand that classes will only present during the school term.  No classes will

            be given on public holidays, unless otherwise stated

7.         I further agree to the full Terms and Conditions.
